Included Health is looking for a motivated RDN experienced in inpatient and outpatient counseling services and telehealth. The ideal candidate has worked with patients both virtually and in person to deliver patient-centered, multidisciplinary, lifestyle education. The chronic care model is a proactive delivery system that includes self-management support, shared decision making, and a team-based approach. The Included Health RDN will receive comprehensive training on our virtual primary care platform to deliver nutrition education for prevention and improvement of chronic conditions. #LI-Remote About Included Health Included Health is a new kind of healthcare company, delivering integrated virtual care and navigation.
